Document checklist before you file an insurance claim

Checklist and documents for filing an insurance claim

Most avoidable delays start with incomplete papers. Use this checklist before the first submission.

Filing early is useful only when the file is complete. Incomplete packs create query loops that feel like unexplained delay.

Identity and policy papers

  • Policy schedule and wording
  • KYC of proposer / patient as required
  • Claim form filled and signed

Treatment papers

  • Discharge summary
  • Doctor prescriptions and investigation reports
  • Final hospital bill with break-up
  • Payment receipts / settlement proof

For reimbursement claims

  • Cancelled cheque or bank details
  • NEFT form if the insurer asks for one
  • Any query replies already given to TPA

Before you submit

Number the pages, keep a full copy with yourself, and send through a channel that gives acknowledgement. Policy Setu can help you spot obvious gaps before the first filing.

Frequently asked questions

Are original bills always required?
Many reimbursement claims ask for originals. Keep clear scans and ask the insurer what format they accept before you send the pack.
What if the hospital delay is causing missing papers?
Write to both hospital and insurer noting the pending document. A dated request helps if the claim later looks incomplete.
